# IOCTL failures causing crashes and freezes in various processes

## Symptoms

- FWK or CPD crashes on Gateway, VSX or Maestro/VSX-Maestro.
- One of the blades enabled: AV/AB/URLF/ZPH, and RAD sends large number of IOCTLs
- We see the following messages in fwk.elg:.
  - `fwk_ioctl_mq_get_reverse_sock: failed to receive reverse socket for pid`
  - `fwk_ioctl_mq_read: failed`
- In SP, we see the following messages on SMO:
  - `lbd_send_bucket_list: members = 0xd00000000, active_members = 0x100200000000`
  - `where values of members and active members are greater than 0x10000000.`
  - `It indicates that the SMO was sending corrupted state to the MHOs.`

This problem was fixed. The fix is included in:
